It should not be understood to constitute any type of diagnosis or healthcare recommendation. In the view of this website, self-administered psychological screening tests may help to enhance self-awareness of one’s own experiences, but cannot give any well-informed recommendation about what should be done about those experiences. What any individual chooses to do — or ‘should’ choose to do — with the results of any given test, is a matter for the individual and should not be dictated by the test itself. If one has any questions regarding any aspect of these screening tests I am happy to discuss them further in my office.
 

Divorce and Custody Disputes and Oompa Loompas

If you become involved in a divorce or custody dispute, a school dispute, or other legal proceedings, I will not be available to provide expert testimony in court. If this were needed, I would encourage you to engage a mental health professional with expertise & training in providing forensic testimony.  This is based on two primary factors: (1) my evaluation would be viewed by the court as biased in favor of you, my client, because we have established a therapeutic alliance, and (2) the testimony may have a negative impact on our relationship. Knowing this, to provide testimony on any clients behalf, would be unethical.  For these reasons and others, I ask all my clients not to involve me in court or school disciplinary appearances of any kind.  That being said, if a client does subpoena me to court I will not participate and I will immediately terminate our relationship.

 

Practice of Non-Discrimination

In my professional practice as a therapist I do not discriminate in accepting and treating clients on any of these basis: race, religious beliefs, gender, age, sexual orientation, physical or mental disability/handicap or ethnicity. This is both a commitment that I have made pursuant to my clients and one meeting accordance with federal, state and local laws and regulations. If you believe for any reason that you have been discriminated against, please bring this matter to my attention immediately.

Various Fees 

In the event your bank check does not clear, a $35 fee will be added to the amount due. Payment is due upon bill receipt. 

 

You have a legal right to obtain a copy of your chart; however, please note that I will discuss with you any reasonable and customary fees associated with copying your chart and its delivery to you. In most circumstances, a summary of your treatment chart rather than the full chart will suffice.

 

All deductibles or co-payments must be paid at the time of our session or, as is mutually agreed-upon, at the end of the month.

 

As a reminder, missed/broken appointments are billed to clients at my customary hourly rate of $160.

Insurance companies do not reimburse for missed sessions.
